We collaborate with community partners and volunteers to provide holistic care for youth, families, and seniors. Brahm Centre works closely with the Agency for Integrated Care and Ministry of Health to support vulnerable elderly with case management and befriending services, serving over 50,000 people through our centers in MacPherson, Newton, Simei, and Tampines.

Role Description

This is a full-time on-site role located in Tampines for a Care Coordinator. The Care Coordinator will be responsible for managing the daily tasks associated with care coordination and case management. Duties include monitoring the well-being of clients, coordinating home care services, providing customer service, and maintaining effective communication with clients, their families, and healthcare providers. The Care Coordinator will also engage in developing and implementing personalized care plans to ensure comprehensive support for clients.

Qualifications

• Experience in Care Coordination and Case Management would be an advantage
• Strong Communication skills
• Excellent Customer Service skills
• Ability to work collaboratively with a team
• Empathy and patience when dealing with clients
• Relevant qualifications in Psychology, Social Work, Nursing, or a related field are beneficial
• Outgoing personality would be an advantage
