• Facilitate the completion of Advance Care Plans (General ACP) for clients.
• Maintain comprehensive and up-to-date records of advance care planning discussions, decisions, and documentation, ensuring compliance with industry best practices, legal and regulatory requirements.
• Oversee locum ACP facilitators' scheduling and workload and provide the necessary administrative support for incentives payment.
• Assist in the recruitment, onboarding and offboarding process of locums.
• Lead and participate in networking and outreach community events, workshops, and seminars to promote awareness and understanding of advance care planning.
• Plan and conduct advance care planning awareness talks for the public.
• Any other related duties as directed by the said reporting officers.

Qualifications

• Possess a Diploma in Social Work or equivalent.
• Trained as a facilitator in Advance Care Planning (ACP General).
• Proficient in local languages and fluent in at least one local dialect.
• Excellent communication and interpersonal skills, with the ability to connect effectively with partners and clients.
• Strong coordinating and facilitation skills
• Able to work independently and collaboratively in an interdisciplinary team environment.
• Demonstrate empathy and sensitivity to the emotional and psychological aspects of advance planning discussion.
