Provide support in the development of integrated models of care, clinical services and projects within the hospital and in the community mainly through capability building, engagements and collaborations with the multi-disciplinary health care professionals, primary care partners, community partners and government agencies.

The incumbent shall be responsible to:

• Assist to develop and implement care models and workflow processes through engagement efforts, collaborations and upskilling of primary care and community mental health partners
• Provide project management and operational support for community mental health related projects including funded programmes to achieve the desired clinical, financial, resource and process outcomes
• Plan and organise training sessions for primary care and community mental health partners including hosting webinars, soliciting feedback and working with trainers to develop e-learning modules
• Prepare, monitor and ensure accurate and timely reporting of progress reports and performance indicators
• Provide meeting secretariat support to the mental health teams, workgroups and committees, both internal and external to organisation

Requirements

• A Bachelor’s Degree from a recognised institution
• Excellent interpersonal, communication and writing skills
• Excellent project management skills
• Able to work with a diverse groups of stakeholders
• Good working knowledge of Microsoft Office Applications (Word, Excel, Powerpoint)
• Entry level graduates may apply
• Contractual until 31 March 2028 (renewal is subject to performance review and funding availability)
