Therapy Assistant - #1089334

Job Responsibilities

• Supporting clinical team in providing assistance to clients in managing their healthcare needs
• Perform tasks according to care plans and operational needs.
• Ensure consistent implementation of rehab program and activities for clients as prescribed by therapists.
• Access electronic documentation systems and maintain up-to-date documentation of clients' conditions and interventions and support development and review of care plans.
• Ensure clients' integration and recommend measures to mitigate risks to the clients.
• Provide inputs to clinical team for the development and review of care plans.
• Support team members on accuracy of documentation of clients' condition, progress, daily activities and care treatments.
• Monitor and initiate interventions for unusual or responsive client behaviours after discussion with clinical team.
• Support team members in ensuring clients' integration in centre.
• Design social, recreational and educational activities to promote independence, health, wellness and quality of life.

Requirements

• Minimum NITEC in Rehabilitation Care or equivalent.
• At least 1 to 2 years of relevant experience in the healthcare sector.
• Able to work independently and in a team.
• Have a passion to provide care to disability adults.
• Possess excellent communication and interpersonal skills.
